Piles, fissure and fistula treatment in Ramamurthy Nagar
Bleeding, pain or a lump while passing stool is common, treatable and nothing to be embarrassed about. Our Kshara Sutra procedure room is led by a surgeon trained in Ayurvedic surgery.
Signs to look out for
- Bright red blood on the stool or in the pan
- Severe burning or cutting pain during and after passing stool (common with fissure)
- A lump that comes out while passing stool, with or without pain
- Itching, discharge or a moist feeling around the anus
- A painful swelling near the anus that bursts and keeps discharging pus (common with fistula)
- A painful swelling or discharging opening near the tailbone (pilonidal sinus)
Piles, fissure and fistula are different problems
Piles (haemorrhoids, Arsha) are swollen veins at the end of the back passage. A fissure is a small tear in the skin of the anus. A fistula (Bhagandara) is an abnormal tunnel between the inside of the anus and the skin outside, usually after an abscess. A pilonidal sinus is a track containing hair near the tailbone. Each needs a different treatment, so an examination comes first. Full details of each condition and procedure are on our para-surgical procedures page.
How we treat it at AyurRemedy
- Kshara Sutra for fistula-in-ano and pilonidal sinus: a medicated thread placed in the track and changed weekly, done as an out-patient procedure
- Kshara Karma for piles: a herbal caustic applied to the pile mass
- Agnikarma for sentinel tags and to ease spasm in fissure
- Medicines, sitz baths, diet and bowel advice to heal and to prevent constipation and recurrence
These procedures are done in our dedicated Kshara Sutra procedure room. Your surgeon explains the procedure, the number of visits and the recovery before anything is done, and nothing starts without your consent.

Questions patients ask
What is Kshara Sutra treatment for fistula?
Kshara Sutra is a thread coated with herbal medicines. It is passed through the fistula track and changed every week, slowly cutting through and healing the track. It is done as an out-patient procedure, and most people go back to routine work soon after.
Do I need to be admitted to hospital?
Most Kshara Sutra and Kshara Karma procedures at AyurRemedy are done as out-patient procedures. Your surgeon will tell you after examining you whether that is suitable in your case.
Is the procedure painful?
Some discomfort is expected; local anaesthesia and pain relief are used where needed. Your surgeon will explain what to expect and how to care for the area at home.
Who does the procedure?
Dr Rasmi Johnson, BAMS, MS (Shalya Tantra), a postgraduate in Ayurvedic surgery with long experience in Kshara Sutra and para-surgical procedures.
